The authors describe the Fernando Fonseca Psychiatric Department and after a brief theoric report about Psychosis early intervention and its prognostic implications, describe more specificaly the PSIC Program (Early intervention pro- gram after a first psychotic episode). After characterizing the initial sample, they describe the patients' evolution during the three phases of the Program evaluation. Finally they present future implications in the development of new projects in this area in the Service, regarding the good therapeutic adherence, the low rate of readmissions and the decrease of Psychosocial level.

Emotions are affective states of short duration, with concomitant vegetative symptoms, triggered by an internal or external perception. There are primary or universal emotions (happiness, sadness, fear, anger, surprise, disgust), secondary or social emo- tions (compassion, shame, guilt, contempt, jealousy, envy, pride, admiration) and background emotions (wellbeing, malaise, calm, tension). Emotions are complex programs of actions shaped by evolution. The actions are completed by a cognitive program, but the world of emotions is primarily a world of actions carried out in our body from the facial expressions and body positions to the changes in the viscera and internal milieu. Anxiety disorders have been described as features of Bipolar Disorder (BD), and Obsessive-compulsive-bipolar disorder (OCBD) may occur in as many as 56% of obsessive-compulsive patients. Mania in Obsessive-Compulsive Disorder (OCD) can occur either as an independent comorbidity or as a result of an antidepressant-induced switch. We report the case of a 38-year-old male with a 3 year diagnosis of OCD treated with antidepressants, admitted due to a manic episode, and describe diagnostic and treatment challenges of this comorbidity.
Since its recognition in the 80s, HIV infection has reached 65 million people worldwide. The presence of the virus in CNS occurs in most patients, increasingly being identified neuropsychiatric disorders associated with infection and / or treatment with ARV. This article intends to briefly review the neuro-pathogenesis and neuropsychiatric disorders associated with HIV infection and treatment with HAART, as well as its therapeutic approach.
Introduction: Karl Kleist primarily described the concept of cycloid psychosis. Leonhard set the basis for the current conceptualization of the disorder describing three overlapping subtypes and Perris developed the first operational diagnostic criteria. Neurocognitive deficits in schizophrenia have been described since the first descriptions of the disorder. Its influence on the functionality and quality of life has been demonstrated in multiple studies . The Measurement and Treatment Initiative Research to Improve Cognition in Schizophrenia (MATRICS ) of the National Institute of Mental Health (NIMH) in the United States was launched to promote the development of a consensus cognitive battery that could be used in clinical trials of drugs to improve neurocognition in schizophrenia. The present article is an analysis of the role of akratic feelings on empathy and self-consciousness. It argues that akratic feelings create a meta-emotional platform that allows the installation of a type of empathic process, which simultaneously contributes for self-consciousness. The article shows in what way akratic feelings are crucial to further understand both ourselves and others.The article begins by describing the nature of akratic feelings and the way in which we can find them at various emotional levels. The second part points out how akratic feelings contribute to empathetic processes and their role in the formation of a meta-emotional platform in which people recognize their opacity. Finally, the article points out how this also contributes for self-awareness, and ultimately for a better understanding of emotional processes.
This paper tries to clarify the concept of emotion, stressing the importance of emotions for psychological adaptive functioning and psychotherapy. This clarification makes use of different characterizations and diferentiations, namely: (a) emotional schemes; (b) different types of emotions; (c) different types of affective phenomena and; (d) emotional functions. The concept of emotion is also related with the concept of psychological needs, stressing the importance of sound emotional functioning to the regulation of needs satisfaction. An innovative model of psychological needs is introduced, showing their importance for both well-being and mental health. The paper ends by claiming that, in psychotherapy, the ability to regulate needs should be taken into account.
