This paper is a reflection upon the meaning and importance of silence in the psychoanalytical relationship. Beginning with the silence in the “normal” relationship between people, we show how silence can be experienced as confortable or unconfortable, and how it can be used to achieve a bigger proximity or distance in the relationship with others. We show these same aspects in the psychoanalytical relationship, and the evolution of the regard towards silence along the development of psychoanalysis. We end, presenting the Nacht’s thinking about silence, who emphasizes its integrative and fundamental role in the psychoanalytical relationship. Thus, only through silence certain affects can be born, and silence allows the patient to internalize the analyst.
Background: Temporality is an essential and organizing element in the experience of being. It is a fundamental element in the coherence of self. In spite of its importance, it is a subject not well investigated, particularly in schizophrenia, where studies havesuggested that alterations of time experience may be a nuclear phenomenon in the psychopathology.Aim: This paper intends to understand how changes in temporality may be related to the psychopathology in schizophrenia. Methods: A selective review of the existing literature has been performed based on Pubmed database starting in 2000, and with a special focus on the classical psychopathology literature. Results and Conclusions: The constructs of time, lived time and temporality are distinguished. Hypotheses are formulated with evidence provided regarding how changes in the experienc...
Introduction: The tension‑type headaches (Ctt) are the most frequent headaches in the general population and those with higher socio‑economic impact, given the high degree of disability they cause. Objective: The authors propose to conduct a review of the available literature on the subject, from a psychiatric perspective. Discussion: Several studies have identified a higher prevalence of psychiatric disorders, personality traits and ineffective coping mechanisms in patients with Ctt, so it is essential to understand this relationship and the impact of these psychopathological factors on this kind of headaches. Conclusion: Their clinical and therapeutic approach is hampered by these and other factors and multiple strategies of pharmacological and psycho‑behavioral treatment have been used on them, however, scientific evidence is still ...
Unlike in children, the autism spectrum disorders (ASD) in adults have not been so extensively studied, with consequent difficulties in diagnosis and management in adulthood, especially in the presence of psychiatric comorbidity. The authors have made a selective review of literature, focusing on ASD definition, its epidemiology, diagnosis and factors that may influence its outcome. The main psychiatric comorbidities in adults will also be focused, as well as its impact in the clinical presentation of psychiatric disorders. Despite the tendency for a progressive symptomatic improvement in adulthood, ASD affect patients during their whole life. Furthermore, it is estimated that they affect a considerable number of patients, making even more relevant a thorough knowledge of these pathologies.
Neurosarcoidosis can present neuropsychiatric manifestations, being a rare form of presentation of sarcoidosis, with a possible large spectrum of clinical manifestations, making the diagnosis more difficult. Neuroimage exams are an essential tool in detecting central nervous system (CNS) lesions, but definitive diagnosis can only be established after biopsy and anatomopathological examination. The authors present a clinical case of neurosarcoidosis, where the first manifestations of the disease were tonic-clonic seizures and psychotic episode.
Introduction: The affective component of pain incorporates various emotions, primarily negative in quality. A great emphasis has been traditionally given to the role of depression and anxiety in chronic pain. More recently, the focus has been directed towards hostility and anger, as fundamental components of the emotional experience of chronic pain. Objective: The aim of this article is to present a literature’s review about the association between chronic pain, anger and hostility. Discussion: Patients with several chronic disorders are characterized by high levels of trait anger and hostility. On the other hand, the manner in which angry feelings are typically handled (anger management style), especially the marked tendency to suppress or express angry feelings, is a particularly important determinant of the chronic pain severity. Co...
The neurological functions of the cerebellum regarding the regulation of balance and movement have been known for a long time. In the last decades the cerebellum participation in cognition, emotional processes and behavior has been progressively recognized specially due to Schmahmann’s studies. The authors of the current paper will briefly review the main aspects of the cerebellum’s “classic” physiology. Afterwards, neuro-psychiatric functions and cerebellar alterations in the main psychiatric disorders - schizophrenia, bipolar disorder, major depressive disorder - will be reviewed in more detail.
