Introduction: Decision Capacity Assessment is one of the functions of consultation psychiatry. It involves the ability to make choices autonomously and arises in a relevant way in clinical practice as a precondition to informed consent.Aims: The authors pretend to review the process underlying decision capacity assessment, as well as other relevant information published in this regard. Methods: Non systematic review of the literature, through electronic search in Medline/Pubmed.Results: The ability to decide can only be assessed in relation to a particular decision, and its outcome is not necessarily stable over time. Similarly, there is no single diagnosis that allows concluding for incapacity, nor dispensing its proper evaluation. It depends on various factors like the thought conte...
Background: Overweight is an increasing problem worldwide. Data from 2008 show that, in Portugal, 60% of the adult population was overweight and 25% was obese. The relation between mood disorders and obesity is well known and about 2/3 of those who search for bariatric surgery have a psychiatric diagnosis, being depression the most common.Aims: We reviewed the relation between depression and obesity before and after bariatric surgery and evaluated its impact in the pharmacokinetics of antidepressant medication and nutrients that influence depressive symptomatology. Methods: We conducted a non-systematic review of the literature published in English between 1988 and 2015, through research in MEDLINE with the keywords absorption, bioavailability, bariatric surgery, obesity, depression, antidepressants. Results: Depression and obesity pot...

We compared the demographics and the clinical characteristics of the drug addict patients admitted to the medical and surgical wards of Hospital Fernando Fonseca referred to the Consultation Liaison Psychiatry (CL) service in the years 1997 and 2004. In this time frame, there was a reduction in the number of referrals, as well as in the number of patients that were actively consuming drugs before admission. A higher percentage of women, of patients from medical wards, and of patients with infectious diseases and HIV positive were referred to the CL service in 2004, compared with 1997. The mean age of referrals, and the rate of those that were followed in drug addiction outpatient clinics or were doing replacement therapy before being admitted were also higher in 2004. Based on the results presented, the role of the CL service in the tr...
Background: Cotard’s Syndrome (CS) is a rare and severe neuropsychiatric condition in which the central feature is the existence of nihilistic delusions. Controversy has ensued about the precise clinical picture Jules Cotard meant to describe and attempts have been made, more recently, not only to clarify the terminology, but also to define different types of this syndrome and explore its biological basis.Aims: We intend to briefly review the terminology, etiology, epidemiology and differential diagnosis of this syndrome, based on two clinical cases. Methods: Bibliographic search conducted through the electronic databases Medline and Gallica (French National Library), consultation of clinical records and direct interviews with the patients. Results and Conclusions: Typically the patients diagnosed with CS manifest the delusional idea tha...
This paper begins with an introduction where the aims are explained: a psychopathological analysis of a Shakespearean character - Othello – followed by the discussion of the English dramatist’s importance in helping us understand madness in the emergent world of Renaissance. The main characteristics of Othello’s personality, which allowed the development of his jealousy delusion, are described. Finally, the conclusions underline the overlap of the symptoms developed by the character with the DSM-IV classification.
Psychological dimension is emphasized in different approaches and languages over migration in a review of its reasons and circumstances, analyzing the person “decontextualization”, his rupture, the transcultural risk and the migratory trauma that may occur. The concept of disease and illness are mentioned as well as the concept of “alterity”, the cultural dimension in the therapeutic relation and specific addressing techniques.
Drawing from my experience as an anthropologist working at the “Consulta do Migrante” at Hospital Miguel Bombarda in Lisbon, this text proposes a short reflection on the necessary dialogue between different fields of knowledge and different forms of relation to the Other, in a clinical setting. Problematizing the encounter with difference in an institutionalized service such as a hospital, the aim is to show the richness of the complex reality implicit in such an encounter between migrants and established health services.
