Coats plus is a highly pleiotropic disorder particularly affecting the eye, brain, bone and gastrointestinal tract. Here, we show that Coats plus results from mutations in CTC1, encoding conserved telomere maintenance component 1, a member of the mammalian homolog of the yeast heterotrimeric CST telomeric capping complex. Consistent with the observation of shortened telomeres in an Arabidopsis CTC1 mutant and the phenotypic overlap of Coats plus with the telomeric maintenance disorders comprising dyskeratosis congenita, we observed shortened telomeres in three individuals with Coats plus and an increase in spontaneous γH2AX-positive cells in cell lines derived from two affected individuals. BACKGROUND & AIMS: The efficacy and tolerability of faldaprevir, a potent hepatitis C virus (HCV) NS3/4A protease inhibitor, plus peginterferon (PegIFN) and ribavirin (RBV) was assessed in a double-blind, placebo-controlled phase 3 study of treatment-naïve patients with HCV genotype-1 infection.
METHODS:
Patients were randomly assigned (1:2:2) to PegIFN/RBV plus: placebo (arm 1, n = 132) for 24 weeks; faldaprevir (120 mg, once daily) for 12 or 24 weeks (arm 2, n = 259); or faldaprevir (240 mg, once daily) for 12 weeks (arm 3, n = 261). In arms 2 and 3, patients with early treatment success (HCV-RNA <25 IU/ml at week 4 and undetectable at week 8) stopped all treatment at week 24. O presente estudo tem como propósito refletir sobre a prática de enfermagem no que concerne à temática da monitorização hemodinâmica. Pretendemos identificar a evidência empírica produzida sobre o comportamento dos enfermeiros perante os alarmes clínicos e, consequentemente, incentivar a adoção de estratégias que promovam um ambiente de cuidados intensivos menos ruidoso. Perspetivou-se um estudo de revisão sistemática da literatura. Selecionámos um conjunto de dezoito bases de dados eletrónicas, tendo recorrido a três idiomas. Progressive multifocal leukoencephalopathy (PML) caused by reactivation of the JC virus (JCV), a human polyomavirus, occurs in autoimmune disorders, most frequently in systemic lupus erythematosus (SLE). We describe a HIV-negative 34-year-old female with SLE who had been treated with immunosuppressant therapy (IST; steroids and azathioprine) since 2004. In 2011, she developed decreased sensation and weakness of the right hand, followed by vertigo and gait instability. The diagnosis of PML was made on the basis of brain MRI findings (posterior fossa lesions) and JCV isolation from the cerebrospinal fluid (700 copies/ml). IST was immediately discontinued. Reported in vitro data implicated soluble CD40 ligand (sCD40L) in endothelial dysfunction and angiogenesis. However, whether sCD40L could exert that influence in endothelial dysfunction and angiogenesis after injury in acute myocardial infarction (AMI) patients remains unclear. In the present study, we evaluated the association of sCD40L with markers of platelet activation, endothelial, and vascular function during a recovery period early after AMI. A variabilidade individual da maturação do ser humano faz que a definição e diagnóstico de atraso pubertário sejam apoiadas, por um lado, em dados estatísticos, por outro lado em critérios bem estabelecidos (tabelas de Tanner). A classificação etiológica, com as consequentes implicações terapêuticas, poderá apresentar dificuldades mormente no que respeita à distinção entre atraso pubertário simples e hipogonadismo hipoganodotrófico – as provas dinâmicas requerem uma interpretação atenta, conjugada com a clínica e, necessariamente, o estabelecimento dum protocolo que permita respostas com valor diferencial. Salientam-se os atrasos pubertários simples pela sua frequência (90% dos casos) e apontam-se linhas gerais da terapêutica desta e das outras etiologias de puberdade atrasada.
