Introduction: Olfaction is frequently affected in chronic rhino-sinusitis with polyposis and has been recognised to have important impact on quality of life. Surgical resolution on cases of maximal medical therapy failure is an option to relieve symptoms, with debates as to how extensive surgery should be. A more radical approach will achieve better disease control with less relapse, but can also compromise olfaction. This decision about a more radical surgical approach should be shared with the patient. Thorough informed consent regarding disease control and hyposmia should be taken.Material and Methods: Literature review and consultation with a board of experts. Results and Discussion: We propose some elements to be included in the informed consent discussion, in order to broadly address the surgical limitations regarding anosmia as a...
Objective: To determine the prevalence of hearing loss in Navy personnel that was reveled between June of 2005 and June of 2006 at the Navy Disability Evaluation Council, categorizing these cases in accordance with: the causes of hearing loss, the year of enlistment in the Navy, the presence on expeditions in the Portuguese African war, the rate, the category, the rank and the degree of disability due to the hearing loss. Methods: A retrospective observational study was performed. Data was collected from the records of the Navy Disability Eval- uation Council and from the records of Otolaryngology consul- tation carried out at the Portuguese Navy Hospital. Results: Hearing loss prevalence is 24%, this sample is com- posed mostly of individuals with noise induced hearing loss, enlisted in the Navy between 1965 and 1970, who were present i...
The cervical cysts hove a behaviour and prognostic that vary according to its source, time of evolution, nature and dimensions. One should consider metastasis of pharyngeal, laryngeal, thyroid, parathyroid, lungs and salivary glands neoplasms.Any swelling in retro and parapharyngeal spaces can cause digestive and airway compromise.Thyroid cysts spontaneous haematomas cases ore rare. The authors describe o case report of a young woman with a sudden set of painless cervical tumefaction causing dyspnea, related to a thyroid node which is exclusively cystic, with sero-hemathic filling, as well as the applied treatment.ln this work it is presented o retrospective of cervical masses cases that occured in lhe Navy Hospital from 2001 to 2005.There were no records of thyroid haematomas within this period.
Introduction: The authors report a complicated chronic frontal sinusitis, resolved with modified Lothrop procedure.Material and Methods: A 39 years old male, had his frontal bone reconstructed after head trauma with fracture of the frontal bone. After this, multiple episodes of recurrent acute frontal sinusitis associated with facial cellulitis emerged. He also had severe headaches due to dehiscence of the posterior wall of the frontal sinus. As there was severe stenosis of the frontal recess he was treated with modified Lothrop procedure. Results: After scarring there was a good epithelization of the frontal cavity, and the inflammatory process resolved. Conclusion: Chronic sinusitis is a known complication of frontal bone fracture. In the presented case the modified Lothrop procedure is a valid and successful choice. Literature review ...
Introduction: Diving has become increasingly popular with time, with an increase of associated pathologies. This issue is especially important in deep diving, where there is less experience and absence of published data. The lack of information is even more concerning when dealing with professional military divers, who are at increased risk, given their higher exposure to diving and the specific nature of activities performed.Material and Methods: We aimed to characterize the nasal airflow of military divers participating in an international diving exercise occurred in Portugal between September and October 2010, and performing daily deep dives. Results: We studied 35 divers, submitted to a daily maximal inspiratory nasal flux measurement (nasal peak flow) and to a nasal congestion evaluation questionnaire, before and after each dive.Co...
Objective: To determine the prevalence of exostoses and osteomata in the Portuguese Navy divers and its relation with the years of diving, age and immersion time.Patients and Methods: One hundred forty nine patients were included in the study. The study group (Group A) was formed by 87divers and the contrai group (Group 8) was formed by 62 individuais that have never dived. A clinical history and otoendoscopy was taken to ali individuais. The exostoses were classified in three grades according to the percentage of obstruction within the externa/ auditory canais. Results: Both groups were uniform in age and sex. There were 86% of divers with exostoses in one or both ears in group A and 43.6% had 1 or more osteomata. With chi-square test it was established a significant difference (p<0.05) in exostoses prevalence between the two groups.Co...
Objectives: Myringotomy with tubes is the most common otologic surgery, and most surgeons prescribe water exposure precautions after surgery. The dynamics of air and water inside the external auditory canal and the transtympanic tube during submersion is analysed and preliminary results are presented.Study Design: Multiphase Computational Fluid Dynamics Model (CFD)Material and methods: A computerized tomography was the base of the outer and middle ear geometry. The multiphase air-water model of Computational Fluid dynamics (CFD) model was then used in the study the inflow of water, and the escape of air through the auditory tube. Results: The multiphase CFD model effectively describes the progression of the fluid front in the external auditory canal, the behaviour of the retained air and the air release through the auditory tube. The mo...
