Positron Emission TomographyIComputed Tomography (PETI CT) scan is considered a last resource diagnostic tool for the head and neck occult primary tumour. Its ability to identify the occult tumour location may lead to a more adjusted treatment, avoiding undesired side e!fects or invasive procedures. The authors study the current diagnostic relevance of PETICT scan in the management of patients with head and neck occult primary tumour based on a clinical case.A 50 year old male presented with a cervical mass. Fine needle biopsy revealed squamous cell carcinoma metastasis followed by upper airway endoscopy, several imaging studies, bronchoscopy, randomized biopsies of the Waldeyer ring, tonsillectomy and whole body PETICT scan. The workup was inconclusive. The treatment was ipsilateral modified radical neck dissection including level I t...
Introduction: Tympanoplasty associated to ossiculoplasty has the objective to reconstruct the tympanic membrane and the ossicular dynamics in order to restore the isolation of the middle ear and of hearing. Several materials have being used to recreate the sound transmission mechanism of the middle ear. The substitution or the reconstruction includes the use of biologic or aloplastic materials.Materials and methods: The authors describe two reconstruction methodologies of the ossicular chain trough type II tympanoplasty making a comparative analysis between: 1) Use of PORP and 2) Use of shaped incus interposed between the stapes and the tympanic membrane. Results: The use of partial ossicular replacement prosthesis (PORP) for interposition between the tympanic membrane and the stapes was used in situations of ossicular erosion or in sit...
Introduction: The authors present their clinical experience in the use of fasciocutaneous forearm flaps for the reconstruction of surgical defects of head and neck.Material and Methods:It was carried out a revision of the patients submitted to reconstruction of surgical defects of head and neck through microvascularized radial forearm flaps in the past 3 years. A clinical characterization of the patients including age, gender, initial pathology, reconstruction location, complications, and final result was made. Results: Ten patients were studied, aged between 35 and 67 years old, 2 females and 8 males, with head and neck oncological pathology in different stages: 1 spinocellular carcinoma in the submental region; 2 spinocellular carcinomas of the mouth floor; 1 parotid gland mucoepidermoid carcinoma; 1 sublingual gland mucoepidermoid c...
Introduction: The authors describe a clinic case of mild ostructive sleep apnea syndrome, whose study has revealed a right parapharyngeal neoformation at the level of the styloid apophysis, adjacent to the skull base.Material and Methods: 58 year old female patient with with a mild obstructive sleep apnea syndrome diagnosis. The patient had been submitted to uvulopalatoplasty in 2008 without clinical resolution. Structurally the patient presented a classe I modified Friedman’s anatomic classification for Obstructive Sleep Apnea Syndrome, with a type 1 Angle’s class and body mass index of 24. The endoscopic study of the upper airway has not revealed morphological aspects to point out. Sequential diagnostic exams were requested to evaluate possible anomalies of the upper airways. Results: Imaging studies revealed the existence of a expans...
Introduction: Carotid blowout syndrome is a rare life threatening complication resulting from head and neck cancer treatment. The main predisposing factors are neck dissection, radiation therapy, local infection, tissue necrosis, vascular exposure and pharyngocutaneous fistula.Materials and Methods: The authors present a clinical case and bibliographic review contemplating the predisposing factors, clinical presentation, treatment and prognosis of this entity. The rupture of the carotid artery was observed on a patient with retromolar trigone carcinoma, exposed to radiation therapy and after surgical treatmentResults: Bleeding was controlled with carotid ligation. Other solutions for preservation of carotid lumen were considered inappropriate or unavailable. In spite of the expected high rate of cerebrovascular events, there were no ne...
