The author puts forward some considerations about the psychiatric practice in the emergency room of a general hospital. Clinical practice in the emergency room should be highlighted by collaborative work between several medical subspecialities, in- cluding internal medicine and neurology. This is particularly true for patients with comorbidity in order to provide quality of care. Finally, the author reviews several clinical cases and suggests guidelines for their management.
We compared the demographics and the clinical characteristics of the drug addict patients admitted to the medical and surgical wards of Hospital Fernando Fonseca referred to the Consultation Liaison Psychiatry (CL) service in the years 1997 and 2004. In this time frame, there was a reduction in the number of referrals, as well as in the number of patients that were actively consuming drugs before admission. A higher percentage of women, of patients from medical wards, and of patients with infectious diseases and HIV positive were referred to the CL service in 2004, compared with 1997. The published bibliography about neuropsychiatric adverse events related to the treatment with interferon alpha in patients suffering from viral hepatitis is reviewed. The authors pointed out adverse events such as mood disorders, anxiety disorders, suicide and deliberate self-harm, manic symptoms and psychotic disturbance. The onset of these psychiatric disorders may be a cause for poor, non-compliance or discontinuation with medical treatment. A good response to psychiatric treatment is observed so that it’s many times possible to maintain medical treatment. A multidisciplinary team in order to facilitate early assessment and treatment of this conditions may contribute for a better prognosis of patients with viral hepatitis.
Pain causes emotional distress and decreases quality of life. A significant percentage of pain patients shows depressive and anxiety disorders. To evaluate the prevalence of anxiety and/or depression in the patients followed at Pain Unit of Hospital Fernando Fonseca, HADS was administered. On the sub-scale of anxiety, 63% of women and 15% of men had "high" scores (cut of point of 11 or more). On the sub-scale of depression, 42% of women and 27% of men had "high" scores. The correlation between the Pain Visual Analogue Scale (VAS) and the HADS total scores was found to be statistically significant.
In the article, some recent studies are discussed, that show the close relationship between pre-frontal cortex and the neurophysiology of bipolar disorder. The most frequent findings in imagiologic studies are hyperintensities in subcortical and periventricular white matter. In the pre-frontal cortex, at the subregional level, there is a loss of grey matter volume, and loss or atrophy of glial cells. One of the areas that is more affected is the subgenual area. Functional studies find less pre-frontal activation in specific tasks. Other studies relate some of the changes to medication. There is a great variability in results, which can be explained by factors such as variability in patients or in methods of research.
Eating disorders should be understood in a multidimensional perspective, emphasizing a biopsicossocial context. In these pathologies it`s the body, in the first instance, that reveals the disease, being in this way the target of the conflict, revealing a disturbed body experience and as a consequence a weak conception of their personal body image. The body image is conceptualised as a subjective image that the individuals form in their own mind, about their body, in relation with differ- ent contexts of life. The intent of the studies is to comprehend the level of body image disturbance, which have concluded that in the majority of the cases, significant changes on perceptive capacity of the patients do not exist. Systemic lupus erythematosus (SLE) is a chronic inflammatory autoimmune disease of unknown etiology, with episodic course. It is characterized by periods of relative quiescence and periods of exacerbations which may involve any organ or system. About a young woman with a clinical delirium state, we revised the clinical neuropsychiatric features of SLE in the literature.
