Segundo a OMS, 36 dos 57 países que enfrentam uma crise de RHS são Africanos. Em S. Tomé e Príncipe uma consultoria efetuada identifica três problemas: Gestão de Recursos Humanos, Gestão de processos de capacitação, e Desenvolvimento Institucional da Escola Superior de Quadros de Saúde Vítor de Sá Machado, atual ICSVSM (Dussault et al., 2010). A partir da Declaração de Paris, foi consensuado que o desenvolvimento de capacidades é um processo endógeno de mudança e apropriação local, e enfatiza que é responsabilidade dos países em desenvolvimento potencializarem a sua própria capacidade nacional. The implementation in Portugal of a model for integrated management of disease applied to the end-stage renal disease, from 2008 onwards, has completely restructured the way of providing care, as well as the payment and follow-up of patients under dialysis. This article describes the steps taken by the Ministry of Health, in terms of the planning, implementation and follow-up of the policy, with a particular focus on the importance of involving the various groups of interest, the leadership, as well as the capacity of negotiation and influence of the Government.
21st Century brings great challenges in the information and knowledge areas. The public health is intensified in Developing and Less Developed Countries on account of parasitic diseases as well as developed countries because emerging and reemerging diseases. In this chapter we analyze the evolution of the portuguese health system (PHP) from the assumption that its strategic nature, integrating observation mechanisms in health, is increasingly closer of the international approaches to performance evaluation. So, we observed two phenomena that have received little attention from scholars and governments: the strategic management of the PHS, on the one hand, and the assessment of its performance on the other. Describing the evolution of the PHS, and in particular its National Health Service (NHS), we seek a closer connection between this reflection and the planning cycle in progress, based on a set of aspects that we consider relevant for the operationalization of an evaluation model for the next national Health Plan (NHP).
The global debate on scientific research policy for human health has been led by World Health Organisation with important contributions from other stakeholders such as Council on Health Research for Development, the World Bank and the Global Forum for Health Research. Recently it has been dominated by the thematic agendas of major global financiers. After more than 40 years of democracy and 30 years of European integration, Portugal has bridged the research gap it had previously. However, when compared to global and European research policies, Portugal still has a long way go regarding investment in research and development. Health Research in Portugal has been managed by the Fundação para a Ciência e Tecnologia and the National Health Institute Doctor Ricardo Jorge, and it has not been a political priority, emphasized by the absence of a national scientific research plan for health, resulting in a weak coordination of actors in the field.
